I have to make a quick correction here: the MS is awakened in a moment of extreme emotional pain and trauma, not necessarily the death of a loved one. It just natural that most of these traumatic moments happened due to the death of a beloved friend or family member. An example of this is Sasuke awakening his Mangekyo not from killing Itachi, but after learning everything the Leaf Village did to him and their clan and turning all the hate he had for his brother into sadness and regret. Another example would be Sarada not witnessing a death, but THINKING that Boruto (someone she loved) was going to be killed.

in mangekyou state, each individual eye have their own dojutsu.

the dojutsu of each eyes can complement each other, for example :
- sasuke's right eye who generate black flames while his left eye control the black flame
- obito left eye can transfer himself to pocket dimmension while his right eye can transfer distant object

the dojutsu can be tottally different in each eyes, for example:
- itachis left eye can generate black flame while his right eye do tsukoyomi

i have no idea what madara mangekyou can do, however i know the fact that madara's brother dojutsu allow him to see anything in slow motion

You forgot to mention that a person can awaken the Rinnegan if they have Eternal Mangekyou Sharingan or regular Mangekyou Sharingan and Senju DNA and Uchiha DNA and/or Six Paths Chakra.
