Im not an engineer or anything like that, but I noticed that the tail rotor seemed very weak to begin with. You can easily see that its wobbly in a clip before the whole debacle took place. God Bless 🙏 his soul and family 🙏

The tracking on the main blade was so far out. Observe the tips of the main rotor blades just before the tail went in to self destruction mode.
It's a wonder it spooled up without the helicopter dancing everywhere.
Vibration probably was the tail rotors downfall.
