ОтветитьWhen they AC jet called the tower for confirmation the runway was clear, they controller could have simply mentioned that the lights were off on the other runway and the runway they were to land on was the only one lit up.

ОтветитьPilots have a very ! severe incident that they WERE aware of and did NOT SAVE the audio (they have to do sth after the landing, pull a lever, or it will be written over with the next flight). There is enough space on the recording device - so the talk AFTER the successful landing would not have cleared the recording, there is more space on it.
It was not only "we forgot about the voice recorder, we were in shock", because the protocoll should kick in at least by one of them. After all they WERE in safety. And of course they knew there would be an investigation.
It was intentional to obfuscate. They knew they had messed up, and it could have been catastrophic. Nor sure maybe what they had done wrong, but they did not want to know, it was a Save-my-ass move. (if they were tired, and it was confirmation bias - then they are not irredeemable. Howerver, there was time to read the NOTAMS. All of it. Even if it is boring and there are several pages).
That intentional action to not secure the recording - not their mistakes - should be reason for terminating them and maybe even putting their licences in jepoardy.
In this case they MUST have noted their mistake, they must have seen the other airplanes (at least the one that illuminated in the last seconds) - almost too late.
On the Captains inner clock it was around 2 or 3 am and he had been awake for 19 hours (and only got 5 hours of sleep before). The First officer had been awake for 12 hours (and also it was 2 - 3 am on his inner clock).
I would think during the flight they have enough time to read the NOTAMS, even if they are several pages long. And even if they are boring.

ОтветитьOne thing I will say as a pilot who has been flying for over 25 years is that the competence and professionalism of air traffic controllers has deteriorated a lot in the last 4 years. This is partly due to a lot of experienced older controllers retiring, and partially due to the degradation of ATC training due to FAA budget cuts.
In particular, controllers used to ride along in the cockpit every so often to see what kind of workload and distractions the pilots are dealing with, and also to see what airplanes can or cannot actually do. This training is no longer provided, and some of the newer controllers may have never even been in a cockpit at all.
I've also noticed that some newer controllers react extremely negatively, to the point of being quite unprofessional, if we inform them that it is not possible/safe for our aircraft to comply with their instructions, especially if those instructions are given at the last moment/too late.
I feel that the program of having controllers ride along on airline flights needs to be reinstated, and that controllers should be required to hold at minimum a private pilot certificate with instrument rating. More opportunities for us pilots to tour control towers would be good as well.
If we continue on the current path, there will be major air disaster, and probably sooner than later...

ОтветитьConsidering there can happen anything that makes a takeoff impossible in a set time frame, rules allowing a plane to use the same runway when there’s another one about to land, 3,5 miles away, is incredibly stupid.
