In 1999 NASA lost a Mars Orbiter just because there was a conversion error in different measurement systems.
It happen when one software measure force in Imperial Pound and other in Metric Newton, the discrepancy between the outcome resulted in a catastrophic failure.
The orbiter reached so close to the Martian atmosphere that it most probably had burned down.
But fortunately it was not the end of Mars missions as in future, after learning from this failure, NASA had been more careful and sent several rovers to Mars.
