So there is no possibility to examine the sciatic nerve at the place where the piriformis syndrome acts up in the pelvis . Not with MRI or ultrasound. Instead of putting a needle into a nerve which can be risky ? Thanks for this useful info.

I recently suffered 3 months with piriformis syndrome. After engaging in strength exercises I concluded this caused piriformis hypertrophy with a sciatic nerve entrapment. No amount of stretching helped, and I did it all. Driving the car was a nightmare. Oral analgesic with NSAID, max allowable dosages, had no effect. Massage and piriformis injection helped temporarily. As a last resort, I tried a Chiropractor. This included very painful deep tissue massage with the elbow. My joints were then systematically popped. I felt a good measure of relief. After 10 days, I am asymptomatic. Who knew!

How do you go about clinically differentiating hip, SIJ, piriformis and disc herniation? You mention piriformis is by exclusion, what about the other conditions that can also can involve the sciatic nerve?
I have a lot of patients who test positive for multiple overlapping provocative test, piriformis syndrome has always been the last thing I usually consider.

Hi Dr. Nabil. I've been watching your videos for a long time and trying to figure out the pain in my heel. Because all doctors have failed to locate or actually believe that I had any pain for over 20 years.
Until I have raised my voiced and shouted at one of the doctors and told him to perform an MRI and he finally agreed. It turned out to be a "Schwannoma".
I had it removed and man what a relief. I can finally use my whole foot to walk. I can sleep easily without waking up at night to the pain. Plus It was so sensitive to the slightest touch.
So maybe you can cover this issue with your viewers that might have the same problem as me.

What about SI joint disfuntion, hip disfuntion bilaterally? I'v had 2 back surgeries and I'm so much worse than I was before. I'm being crushed from my lower back to my feet.

Thanks for all your contents. I thought that the piriformis was an external rotator when hip is extended but an internal rotator when hip is flexed. I learned that stretching it would be with flexed and adducted hip but with external rotation, not internal rotation. What do you think about this?
