In Sims 3 I believe you can get plastic surgery at the hospital. There is a chance of it either being successful or a failure depending on the type you go for (cheap increases chance of failure, expensive is most likely to succeed).

Former sims2 developer confessed in 2020 that they were on drugs and very high when they made Sims 2 everything makes so much sense now
