Who’s Doing the Rectal Exam?

If the attractive Muslim woman was giving me the rectal exam I would be one happy camper.

I don't see the value of a DRE. Yea you can get a FOBT stool sample, but if a GI bleed is so occult that there is any question on the matter, there is no urgent rush to get the sample. Waiting till they go the bathroom to get a sample is more comfortable for everyone. It doesn't even make sense for fecal impaction, the chance of getting a finger up there far enough to get some kind of manual traction on a hard stool is virtually impossible.

If I was the GI fellow I totally wouldn't turn up in internal medicine for the rest of the shift.

Imagine consulting a specialty trainee just to do a physical examination
Its a waste of their time

They're paid to make decisions about patient care not to do the basic data gathering.

The FY1. The answer is always the FY1. The relief on their face when I can tell them that actually that patient moved their bowels this morning, it just wasn't charted. There was only one person in that wrd round who was gonna be gloving up and the FY2's are breathing a vast sigh of relief that that year of their life is now over.

I've recently had a patient who called for help and for the police when I did a DRE on him. I was a bit shocked as I am a very gentle and small person. He then started laughing his ass off saying he wished he could have seen my face if he wasn't turned with his butt towards me.

Haha true saying that if you don't put your finger in you will later be putting your foot in. That being said, if you have visibly seen melaena come out of a patient, you don't necessarily need to do a DRE or a faecal 'occult' blood test to prove it 😊

Lol we also sometimes try to get ourselves out of doing rectal exams in the veterinary field. But not for the reason of social awkwardness--instead, the risk we'll get bitten and/or the chance we'll find full anal sacs that need to be expressed (and those are FOUL--worse than any other bodily fluid in my opinion). Good thing humans don't have anal glands!! That would require rectal exams to be commonplace...and lots more social awkwardness to go with it.

I’m in ER and I literately ask myself this scenario: if I don’t do the rectal exam who will?

We've had a patient with melena present to emergency medicine but they were also complaining of chest pain (angina due to anemia) . So while they were pumping him up with blood they called a cardio consult who, and I shit you not, recommended "Rectal digital exam performed by surgeon or gastroenterology". I mean I get it that it's icky, but you basically wrote down on a medico-legal document that you did not perform a very basic examination.

Aaah the feeling of warm poop sorounding your finger after a mighty push in the anus...never gets old.

On PICU I got called to the ED to see an 11 day old that was severely dehydrated with concern for hirschsprung’s. ED doc, surgery team, my transport nurse, and handful of med students were all discussing the plan, and when rectal exam was brought up all 10 faces looked at me simultaneously. Unfortunately the exam was indeed concerning for hirschsprung’s, and anyone that knows the exam finding understands why it’s extra unfortunate.

Whenever I have a rectal exam I always say "shouldnt you at least buy me a drink first?" and they laugh like that's not the thousandth time theyve heard that joke.
