That first knot was just two knotless knots pulled together and the tag ends were trimmed too short, when that's been in the water any decent amount of time it will likely pull apart.

its very important how u tighten the fg at the end, this was not a good eg of how u tighten the main part of the knot and the rizutto finish. u should pull the main knot wrap so it grabs, not let it bunch up, then tighten rizutto, then put lots of pressure on both.

I really like how clear your knots are shown. That being said I think your blood knot was tied incorrectly. The tag ends should be going opposite directions when tied correctly , not the same side as shown.
