To start: instead of stopping or slowing down the ship's forward progress on the night of April 14th, the crew was ordered to plow through the night, despite increased warnings of ice in the area from the Californian (The warning was disregarded as Jack Phillips, the radio operator on duty, was busy with personal messages for the passengers) all because the ship's higher ups were determined for the ship to break all speed records on it's voyage from England to America. Also, while the ship's officers and lookouts had binoculars, a crew member had left the ship some time before the sinking, unwittingly taking the binoculars with him; leaving the lookouts with nothing to help them spot the iceberg in time;

And the sinking made things worse; putting aside how there was not enough lifeboats; The ship's architect, Thomas Andrews, failed to communicate crucial facts to the officers, such as how the lifeboats could've been launched beyond their full capacity, and many men died because some officers mistook the order "Women and Children first" as "Women and Children only".

Worst decision was to treat Germany lightly just a years later, ignore the fact that they were obviously building an army (which they were bound not to).
