Honestly, that doesn't make much sense to me. Since his shikai can literally make her fight the fucking air while he chills and eats chips, he shouldn't get exhausted. He stomped everyone TOGETHER during the Karakura town fight, and no matter how much we love Unohana she isn't more of a threat than THE REST of them combined (which he fought).

Unohana can exhaust Aizen meaning at least in hee Bankai Aizen saw her as a threat which makes sense given how he also treated a full power Kenpachi, arguably she may even be eqaul to him in both their Shikai, but on her “seeing through” Kyoka Suigetsu all she said was that she noticed something felt off, probably reffering to one how someone as powerful as him would be killed by anyone who wasn’t her or Yamamoto, and two how Aizen probably didn’t make the illusion as detailed as he should have.
