The number of these videos where at least one of the causes was "people not using calls correctly/clearly enough" is honestly really upsetting. That seems like it would be one of the easier things to do and yet I keep seeing stories where it doesn't happen.

I was on Orangewood Ave, in Garden Grove, CA when this happened. Too young to remember, though. I was born on January 21, and was a month premature. Feb 1 was the day my mom was finally able to leave the hospital with me, and says she remembers the live coverage in the hours afterwards saying it “had to be a bombing, because of the Gulf War” before the details of the second airplane came to light.

I was a air traffic controller at Los Angeles Air Route Traffic Control Center when this happened. One of my close colleagues, who was on my team, died on SKW Sky West. He was in his early 30s, married and a father of two very young children. His name is Scott Gillham. He was a good controller and a very nice guy. I had just ate lunch across from him a few days before this accident at our facility’s cafeteria. His funeral was packed with controllers and very emotional. It hit us hard.
The tower controller was relocated to LAS Tower in an administrative job. She never controlled again. Her over the shoulder review that you mention, from my experience working 25 yrs controlling at both the Center and later LAX radar approach, was not a good review to have that many mistakes. Typically one might have 1 or 2 phraseology mistakes and nothing more. I retired from So Cal TRACON as a supervisor and if that was my employee, I would have utilized remedial training. Sad situation for both crew, passengers, and air traffic controllers,

This is absolutely insane how this airport was set up and worked. Having 1 person responsible for so much over 8 hrs or whatever their shift is is pure insanity. Being hyper aware every minute of every hr is impossible for 1 person to do. So sad and so simple to have been avoided. SMH

I feel so bad for the air traffic contoller. The way you tell the story I can almost feel her confusion and feeling overwhelmed. I get so bogged down at work sometimes that little things just slip by me. I can't imagine the horror she must have felt. And having to live knowing her mistake cost so many lives. I don't know if I could live with myself. This is so tragic. The Boy in the wing seat makes me so sad. Have kids his age and can't imagine the horror that poor boy felt.

I was a pilot for SkyWest on the metroliner hired a year or so after this accident. Still remember taxiing into position on a 45° angle as a precaution.

Dear Petter, you're amazingly intelligent and hardworking at the same time. You make such complicated information so vividly understandable as if you were present in the aircrafts and the air control tower all at the same time. You separate each thread of information with such zeal and skill that it's easily comprehensible for the audience. The disaster would long be remembered as human error (90% ATC 10% other factors). In the late eighties I was in the insurance game selling insurance, employee benefit plans - I remember bidding a company near the big donut, close to LAX- they sold all that fire retardant cloth for seats & Airline Interiors... I got a real education, I didn't get the business I wasn't competitive enough price wise it was very small carve out group... fascinating company, I met with one of the female owners she was quite interesting...LAXVESPA-LOSANGELES the insurance business just got absolutely too crazy, almost impossible to find stable pricing - so now I sell crap on the internet... on various platforms...

As someone who isn't a pilot or ATC I would assume that lights which blocked the view of the runways, even partially, would be ground for stopping traffic operating on those runways at the very minimum. It's such a clear and obvious safety issue its hard to believe they chose to keep using that runway. And its not like there aren't a hundred different temporary solutions which could have prevented this accident, including just turning off that set of lights until they could be fixed.

While it is sad that so many people died a horrible death, and that a lot of people were seriously hurt. It is a good that they did not let incompetence stop the air traffic controller from having a good career.

And it is very good to hear that the final report blamed everyone besides the air traffic controller, blaming her would be sexist

Good analysis of this accident. Non-aviators might be surprised that the landing aircraft didn’t see the small one but just after dusk and given the situation it doesn’t surprise me at all.
Although it’s not a factor here, the American way of giving a “clear to land runway XXX” when still way out is completely contrary to the rest of the world where landing clearance is only given by the tower controller when the runway is clear.

What a terrible tragedy, I've never fully understood why ATC seem to be always undermanned.

Sure adding just one pound/ dollar / euro to the cost of a ticket would provide more staff

I was there that night flying S/O on the 747, doing our preflight for our flight to HNL. We parked on the south complex and I remember opening our cockpit escape hatch to try and see anything. We couldn’t really see much. I always wondered what happened. Thank you so much. Great video.

You made it seem like Candela and Lumens mean the same thing. You said “100 Candela or 1257 Lumens”. Like someone might say they are traveling at 100kph or 62mph. It’s 35c or 95f.

These two measurements do not measure the same thing. Lumens is the total light output in all directions. Candela is the light output in one specific direction. Candela is more of a measure of distance the light can be seen and lumens is total light output.

For instance. A flood light and a spot light can be the same lumens but the spot light will have a higher candela measurement because its light is focused into a tighter beam; making it visible further away.
