Add the fact that anything bigger than 450 KT's is going to be large and take up all the payload of most intercontinental missiles, then suddenly, an adversary would need hundreds of nuclear bombs to destroy one major urban metro area if everyone's hiding underground. So actually, with current stockpile levels, there's good chance like 90% of the population would survive an initial exchange. So good news and bad news; Nuclear war is fairly survivable, but it becomes more likely because it's survivable.
