In Algeria we say "Gynécologie obstétrique" or "Gynéco- obstétrique" for short , we study in french though

In South Africa it's also Obs and Gynae lol

In germany people call them „Gyn“. The why is a ü sound. Or we call them „Frauenarzt“ which means womens doctor

Not sure if this was standard thing around, but in my med school/or just my batch mates in general just used OG. Simple as it can get... 🤷🏻‍♂️

The way the uk says it sounds like five. Also while it probably true obgyn in the us is are only letters so i wouldn't consider callling it syllables, but i understand why it would be
