ОтветитьThank you for giving Lidocaine prior to the Propofol. I'm still amazed that all providers don't do that. I just had an EGD and forgot to ask prior to induction and the last thing I remember is the stinging of the propofol. Also do you complete a surgey from start to finish without taking a lunch break? I had a 3 hour spine surgery and found the anesthesia associate, an anesthesia provider I had never heard of was switched out mid case and then returned an hour later. I assumed for lunch. I was pretty shocked that a large teaching hospital was using anesthesia associates during a complex spine surgery.
ОтветитьHold up... 100cc of propofol for 90$, but 20cc for 6.20$. So, buying 5x20cc (100cc in total) would cost 31$ instead of 90$. Savings opportunity? Edit : I think I get it, the graphics says 100cc for 90, but the bottles are 1000cc, not 100cc.

ОтветитьIsn’t methadone going to block the action of the other opioids/opiates? Methadone has a relatively mild pain management profile when compared to morphine, oxycodone, fentanyl, etc., so I’m also curious why methadone would even be bothered with. It had a long half life as well - further complicating the matter. Perhaps in opiate naive patients things are different - but I would think oxymorphone or OxyContin would be a better choice.

Except my bones are equivalent to a post menopausal woman so they broke out, got infected and i had multiple blood infections which ended up attacking my heart which lead to two open heart surgeries to repair my mitral valve and replace my aortic valve yeah it was bad..

Also does the hospital charge the patient more for these medications than they pay the wholesalers?

My experience with them is that epidurals can make it difficult for patients to wake up after surgery, particularly if they have naturally low blood pressure.

ОтветитьHospital pricing really doesn't matter, we as non-doctors don't care about what hospitals pay for medicine.
What, hospital's pay wholesale or at a 20%-50$ mark up buying from the pharmaceutical companies? Or some sort of discount via buying group.
Then cost for patients is 500%-5,000% mark up on that because that is what the hospital want to claim to the insurance companies so they make money.
U.S. health departments, hospitals, insurance companies, pharmaceutical companies are the greediest people in the world.
And also, half the medication left over is just squirted into the sharps bin.
I'd rather stay home and die than pay for the new MRI machine for the hospital.

My question is what do you do for patients who wake up during surgery and what causes it. I woke up during my shoulder surgery i was staring at the tape over my eyes. All i heard was the noise of the oxygen machine. I heard a lady ask me if i was awake. I knew not to move. I was like fully awake but felt no pain. I started blinking because i was scared to try to talk with a tube in my throat. The next thing i knew i was out.
ОтветитьWhen i was a teen an oral surgent wanted to charge my family $128 an hour for Anesthesia for my upper wisdom teeth. We couldn't afford it. My insurance only paid for the Anesthesia for the bottom ones. The surgent kept pushing for my family to pay for it. We had to wait and rasie the money for it. A few weeks later i was having a cavity filled and the dentists said he would pull the top ones. I told him we couldn't afford it. He ran my insurance and paid for the local Anesthesia. I was soo relieved i didn't have to pay over $300 for Anesthesia. I had no complications from the top ones pulled. I was back on my feet in like 3 hours

ОтветитьWould the methadone not impair the effect of the fentanyl, by pushing it off the opioid receptors? I ask this as someone who takes methadone as maintenance. Obviously a surgery is a lot different than day to day drug interactions. I do not claim to know anything haha, im just curious

The difference in the amounts of Medications compared to Veterinary medicine is astounding. I cant recall the last time we used more than a few mls of Propofol for a single patient, on the other hand, most of our Operations are done with Ventilated-anesthesia using Isoflurane. But still, 400 mls of Propofol is probably more than we use in a whole year, you use it on one patient.
